Recertification must be completed on a biennial basis. Your certification will expire 2 years from the date you pass your certification exam or from the date your recertification application is approved by CHP. The certification expiration date is listed on your member ID card and certification letter. It is the member’s responsibility to keep their certification current.

Complete a secure online application from the Members Screen. From the CHP home screen, log in using the user id (email address) and password, click on the blue Online Application button and follow the detailed instructions.
The following documents should be attached to your online application. The document upload will be completed during the online application process.
This completes the online application process. You do not need to mail or email documents if they have been sent with your application.
If you were unable to upload the required documents to your online application, email the documents to info@chpcertified.com. Please include in the email your full and that you have submitted an online application, but you were unable to attach the required documents.
The recertification application fee can be paid in three ways. During the Member Registration process in Step 1, using the blue E Payment button or by mail using a bank check or money order. If the member did not make the payment during the Member Registration process, use one of the options below.
E Payment Button- From the CHP home screen, log in using the user id (email address) and password and click the blue E Payment button, select the certification type, payment amount and follow the directions provided.
Bank Check or Money Order- A bank check or money order should be made payable to CHP Corp and mailed to the address noted on the Contact Us tab on the home page of the website. Processing of your membership application will not be complete until the payment is received.
$160.00 for a 24-month certification.
If you are not working in the healthcare industry for 12 months and would like to recertify your certificate you are required to complete Continuing Education. This includes written proof of 10 Continuing Education Units (CEU) or 60 clock hours in the healthcare industry.
There are numerous options for a member to obtain the CEU credits, however, they need to be specific to your certification type and recorded on a CHP Personal Development Log. The Personal Development Log can be found on our website, in the Members section, on the blue View and Print Forms button. CEU hours completed online, with third party continuing education partners, can be submitted on their hourly summary form. A Phlebotomy Technician Certification cannot be recertified unless you are working in the healthcare industry completing venipunctures.
